- Collaborate with Physician Advisors: Collaborate with Physician Advisors to identify educational opportunities and ensure that all team members have the necessary skills and knowledge to provide high-quality patient care.
- Document Training Processes: Collaborate with the Corporate Learning Specialist to ensure that new processes are documented for training purposes, ensuring that all team members have access to the information they need.
- Subject Matter Expert: Serve as a subject matter expert on Care Management applications, ensuring that all team members have the necessary skills and knowledge to use these applications effectively.
- Maintain Compliance: Maintain compliance with all Bayfront Health policies and procedures, ensuring that all team members are held to high standards.
- Process Improvement: Participate in process improvement activities as assigned, ensuring that all team members are engaged in continuous learning and improvement.
- Positive Attitude and Professional Appearance: Always demonstrate a positive attitude and professional appearance, setting a positive example for all team members.
- Resource for Team Members: Act as a resource for team members, providing guidance and support as needed to ensure that all team members have the necessary skills and knowledge to perform their jobs effectively.
- Education/Training: Graduate of an approved school of nursing or Master's degree from an accredited school of Social Work, Mental Health, or Psychology is required or related field.
- Licensure/Certification: Florida RN license required and maintained current, if graduated from an approved school of nursing. BLS/Healthcare Provider Certification within 90 days of hire. Maintains Florida state (Chapter 491 Florida Statutes) licensure as a Licensed Clinical Social Worker or Licensed Mental Health Counselor if licensed in these fields or obtains licensure within two (2) years of hire.
- Experience: Three (3) years of experience in acute hospital care management, care coordination and utilization management. Effective written and verbal communication skills. Excellent presentation skills.
