Job Summary

The Registered Nurse (RN) is a key member of our team, responsible for planning, organizing, and directing home care services with a focus on community health education. The ideal candidate will have a strong commitment to providing quality care to our patients and their families.

Key Responsibilities
  • Conduct initial assessments of patients and families to determine home care needs, including physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Counsel patients and families on meeting nursing and related needs, and provide health care instructions as appropriate.
  • Identify discharge planning needs and implement plans prior to patient discharge.
  • Act as a case manager when assigned, coordinating patient care for assigned caseloads.
Requirements
  • Associate's or Bachelor's degree in nursing, with one to two years of recent acute care experience in an institutional setting.
  • Current RN licensure in the state.
  • Basic Life Support (BLS) certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking and standing, with ability to lift, position, or transfer patients, and lift supplies and equipment.
  • Visual acuity and hearing to perform required skills.
