Care Coordinator Leader

Cogir of Tigard is seeking a highly skilled Memory Care Director - LPN to lead our memory care team in providing exceptional resident care services. As a key member of our community, you will be responsible for training and supervising staff, supporting care plan execution, and maintaining communication with residents' families.
We offer a competitive salary range of $85,000 - $110,000 per year, based on experience and qualifications. Our comprehensive benefits package includes health, dental, and vision insurance, 401(k) with company match, paid vacation and sick leave, and more.
Key Responsibilities
- Train and supervise all memory care staff to ensure high-quality care services.
- Support the implementation and coordination of resident care services, including medication management and daily living activities.
- Maintain open communication with residents' families and provide emotional support as needed.
- Collaborate with the Lifestyle team to develop and implement engaging programming that meets each resident's interests and needs.
Requirements
- A current nursing license (LPN/LVN) and a degree in gerontology, nursing, or a related field.
- A minimum of 2 years of dementia experience in a supervisory role and 3-5 years of experience in assisted living and memory care.
- CDP certification and knowledge of various computer systems, particularly Excel and Word.
- Excellent communication and teamwork skills, with the ability to motivate and support staff.
