A skilled Licensed Practical Nurse (LPN) is needed to provide high-quality, patient-centered care to individuals in their homes. As a Home Health LPN, you will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
As a Home Health LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will work in a fast-paced, dynamic environment that requires strong communication and problem-solving skills.
Responsibilities- Implement the nursing care plan for each patient, providing skilled nursing services, treatments, and diagnostic and preventive procedures as assigned.
- Initiate preventive and rehabilitative nursing procedures as appropriate, observing signs and symptoms and reporting to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
- Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Evaluate with the registered nurses the effectiveness of the LPN's/LVN's nursing service to the patient and family under the guidance of the registered nurse.
- Maintain accurate and complete records of observations, treatments, and care of patients.
- Current unrestricted LPN license in Florida.
- CPR certification.
- TB test.
- AHCA Level 2 background check and Health Statement/physical clearing to work.
- Minimum of one (1) year experience in nursing, preferred.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
- Proficient wound care skills.
- Bi-Weekly pay with direct deposit.
- Free continuing education.
