Nurse Care Coordinator

At VNS Health, we are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This is a unique opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Collaborate with interdisciplinary care teams to ensure residents receive comprehensive care.
- Provide health-focused outreach to seniors who may benefit from supportive services.
- Implement health promotion activities, including health screenings, assessments, and referrals.
- Make telephone calls and home visits to assess residents' needs and support successful community living.
- Refer residents to appropriate health services based on assessed needs.
- Provide healthcare advocacy and liaison services during times of transition.
- Participate in quality assurance and education programs as requested.
- Document all nursing interventions as required.
- Current registration to practice as a Registered Professional Nurse in New York State.
- CPR/BLS Certification.
- Associate's Degree in nursing required; Bachelor's Degree preferred.
- Minimum of two years of experience working as an RN in LHCSA or CHHA.
- Prior experience working with an aging population and providing clinical assessments of their health needs.
