Pharmaceutical Care Specialist

Position Overview: This role reports to the Pharmacy Services Supervisor or their designated representative. The Clinical Pharmacist plays a vital role in the development, execution, and sustainability of pharmacy services aimed at enhancing pharmaceutical care. This position actively engages in clinically focused services within the designated scope of practice, demonstrating a strong commitment to delivering exceptional customer service experiences.
Core Responsibilities:
- Medication Order Verification: Ensures the accuracy and appropriateness of medication orders upon physician request by:
- Thoroughly reviewing orders for completeness and compatibility.
- Accurately transcribing medication orders into computerized patient profiles.
- Clarifying orders when necessary and ensuring effective communication of corrections.
- Verifying physician orders, including intravenous orders, for accuracy before dispensing.
- Utilizing electronic medical records to prioritize order verification and clinical follow-up.
- Suggesting formulary alternatives to prescribers when non-formulary items are requested.
- Medication Preparation and Dispensing: Responsible for preparing and dispensing medication orders by:
- Filling various medication forms with precise dosages.
- Using aseptic techniques to prepare intravenous admixtures.
- Compounding specialty solutions while adhering to established guidelines.
- Ensuring proper labeling and prioritization of compounded sterile products.
- Conducting checks on controlled substances and repackaged medications.
- Maintaining efficient inventory levels and providing constructive feedback to staff.
- Clinical Services Participation: Engages in medication management and stewardship to optimize pharmaceutical care by:
- Modifying treatment durations and adjusting medication dosages as needed.
- Discontinuing or adding medications based on patient needs.
- Recommending and monitoring laboratory tests.
- Reviewing patient records to ensure appropriate drug therapy.
- Identifying and addressing issues related to patient compliance and drug interactions.
- General Clinical Duties: Performs various clinical responsibilities including:
- Providing clinical dosing services and reviewing patient medical records.
- Conducting in-services for clinical staff and documenting clinical notes.
- Participating in interdisciplinary rounds and providing drug information services.
- Attending clinical meetings and educational sessions.
- Support and Development: Contributes to departmental policies and procedures by:
- Assisting in medication usage evaluations and reporting medication errors.
- Leading special projects and participating in staff orientation.
- Conducting inspections and maintaining accurate records.
Qualifications:
- A bachelor's degree in pharmacy, a master's degree, or a Doctor of Pharmacy degree from an accredited institution.
- A valid pharmacy license or eligibility for licensure in Indiana.
- Preferred: A minimum of two years of hospital pharmacy experience or completion of a pharmacy residency.
- Required: BLS and ACLS certifications for full-time staff.
