Job Summary

As a RN Case Manager, you will play a vital role in ensuring patients receive seamless care and support throughout their hospital stay. This part-time weekend position requires a skilled and compassionate nurse to assess, develop, and implement discharge plans that meet the unique needs of each patient.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
  • Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact patient outcomes.
  • Participate in the utilization management process to ensure efficient use of resources and optimal patient care.
  • May also be responsible for case management processes, admission processes, and liaison work with post-acute providers.
Requirements
  • Graduate of an accredited school of nursing or equivalent education.
  • Three years' acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
Work Environment

This part-time weekend position is located at SSM Health St. Mary's Hospital - St. Louis, MO. The ideal candidate will be able to work independently and as part of a team to provide exceptional patient care.
