SSM Health is seeking a skilled RN Case Manager to join our team on a part-time weekend schedule. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay and beyond.

Key Responsibilities
  • Assess patients' discharge planning needs and develop plans to ensure safe and effective transitions to next level of care.
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge.
  • Ensure compliance with Condition of Participation (CoP) discharge planning requirements.
  • Identify patients at high risk for readmission and develop plans to prevent readmissions.
  • May also be responsible for utilization management, ED case management, admission process, and liaison with post-acute providers.
Requirements
  • Graduate of accredited school of nursing or education equivalency for licensing.
  • Three years' acute care experience.
  • Frequent keyboard use/data entry.
  • Registered Professional Nurse (RN) licensure in Illinois, Missouri, or Oklahoma.
