This part-time weekend position requires a skilled and compassionate nurse to assess, develop, and implement discharge plans for patients, working collaboratively with the interdisciplinary team to ensure a smooth transition to the next level of care.
Responsibilities- Assess patients' discharge planning needs and develop individualized plans to meet their unique requirements
- Collaborate with the healthcare team to identify and address potential barriers to discharge
- Develop and implement strategies to reduce readmissions and improve patient outcomes
- Participate in quality improvement initiatives to enhance the discharge planning process
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Graduate of an accredited nursing program
- Current RN licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin
- Three years of acute care experience
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
