Key Responsibilities
  • Collaborate with our interdisciplinary team to develop and implement effective care plans that meet the unique needs of each patient.
  • Provide medical oversight and guidance to our clinical staff, ensuring that they have the necessary resources and support to deliver exceptional care.
  • Work closely with our IDT/IDG team to ensure that patients receive comprehensive and coordinated care, including contractual services and symptom management.
  • Certify patients for admission and re-certify them for continued benefits, ensuring that they meet medical criteria for admission and continued care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Serve as a medical resource and expert for our team, providing guidance and support as needed.

Qualifications and Requirements
