Job Description:

HopeHealth is seeking an Access Center Coordinator to join our team, responsible for delivering exceptional patient-centered care and providing timely and responsive customer service. As a key member of our Access Center team, you will play a vital role in processing referrals, communicating with internal teams, and ensuring seamless patient care.

About the Role:

We are looking for an experienced professional with a strong understanding of hospice reimbursement, levels of care, and electronic health records. The ideal candidate will have excellent communication and problem-solving skills, as well as the ability to work independently and collaboratively within a fast-paced environment.

Responsibilities:

  • Provide exceptional customer service to patients, families, and healthcare professionals via phone, email, and in-person interactions
  • Process referrals accurately and efficiently, working closely with internal teams to ensure seamless patient care
  • Communicate effectively with patients, families, and healthcare professionals regarding patient status and progress
  • Collaborate with the Revenue Cycle team to ensure timely and accurate payer information is entered into the Electronic Health Record

Requirements:

  • High school diploma or equivalent required; associate degree, medical assistant, or medical secretary background preferred
  • 2+ years of related experience in a healthcare setting, preferably in customer service or coordination roles
  • Excellent communication, organizational, and problem-solving skills
  • Ability to read and comprehend instructions, correspondence, and medical terminology
  • Proficient computer skills, including Microsoft Office and electronic health records software
