About the Role

Mountain View Hospital is seeking a highly skilled and dedicated Non-Clinical Care Coordinator to join our team. As a key member of our population health team, you will play a vital role in implementing and maintaining successful quality initiative efforts in our clinics.

Key Responsibilities
  • Work closely with the population health team and clinical care coordinators to implement new processes and procedures within the clinic to promote quality patient care and positive patient outcomes.
  • Identify and track care gaps, and develop strategies to close them.
  • Regularly outreach to patients to promote preventive care and services.
  • Act as a liaison between the provider and the patient to coordinate care and ensure seamless communication.
  • Collaborate with referral coordinators and specialists to integrate recommendations and improve patient outcomes.
  • Manage and track care gap reports, and provide regular updates to the team.
  • Participate in webinars, conferences, and education meetings to stay up-to-date on best practices and industry developments.
Requirements
  • High school diploma or equivalent.
  • Experience in outpatient care settings, preferably with population health experience.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, providers, and administrative staff.
  • Ability to use Microsoft Office, Word, and Excel, and adapt to new software systems quickly.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage a heavy workload.
