Key Responsibilities:

  • Conduct comprehensive assessments of patients' physical and psychosocial needs.
  • Develop and implement individualized care plans in collaboration with the Interdisciplinary Group (IDG) and healthcare providers.
  • Coordinate care among IDG members and external healthcare providers to ensure seamless delivery of services.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients.
  • Evaluate and coordinate the need for services in accordance with the IDG plan of care.

Requirements:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience.
  • CPR certification.
