Job Description:

The Licensed Practical Nurse in Long Term Care provides comprehensive patient care services as directed. This includes measuring, evaluating, administering, and documenting patient care. The LPN contributes to patient care plans and informs the Registered Nurse when necessary. They provide direction and assign duties to Certified Nursing Assistants based on their skills and abilities. The LPN actively responds to patient, physician, and family requests, advocating for Patient Rights and Patient Privacy.

Shifts:

The ideal candidate will be available to work Sunday night shifts, with the possibility of rotating Thursday, Friday, or Saturday night shifts. Tuesday and Wednesday day shifts are also required, but Sunday night is the primary need.

Essential Duties and Responsibilities:
  1. Medication administration, care management, and direct patient care, including bathing, dressing, and feeding patients, and assisting in examinations and treatments.
  2. Ability to lift, walk, carry, and stand.
Skills and Abilities Required:
  1. Oversee total patient care and work directly with other staff to ensure the provision of the highest standards of care.
  2. Compliance with the State of Nevada's Nurse Practice Act, Hospital policies, and rules/regulations.
  3. Ability to organize and prioritize work to maintain a harmonious daily workflow.
  4. Check, pour, administer, and document medications at appropriate times, monitoring effects and side effects.
  5. Work with intravenous medications and administer all routes of medications per Nevada State Board of Nursing criteria.
  6. Administer and record treatments as ordered, monitoring progress toward expected outcomes.
  7. Admit, transfer, and discharge patients as required.
  8. Report changes in condition, laboratory results, and unexpected occurrences to the RN and the physician.
  9. Accurate documentation, reporting, and in-depth nursing observation to ensure continuity of patient care.
  10. Daily evaluate patient care needs and condition changes, implementing changes in care delivery as warranted.
  11. Consult with the RN in implementing such changes.
  12. Assure that Patient Rights are upheld.
  13. Transcribe physician orders and communicate changes to other members of the team.
  14. Assure that Labs, X-rays, etc., are completed and notify the attending physician of abnormal results.
  15. Assist as necessary with patient care.
  16. Maintain cleanliness and order on the unit.
  17. Participate in and satisfactorily complete the Orientation Program and Annual Review Days.
  18. Attend inservices and meetings as required.
  19. Pursue thirty (30) contact hours of continuous education, as required by the State of Nevada for re-licensure.
  20. Report deviations in quality of care to Supervisors as outlined by current nursing standards.
  21. Actively participate in Quality Improvement activities.
  22. Maintain working knowledge of hospital-wide order entry.
Education and Experience Required:
  1. Graduation from a recognized Practical Nurse Program.
  2. Current licensure by the Nevada State Board of Nursing as an LPN.
  3. IV certification within one year of employment.
  4. Neat, well-groomed, professional appearance.
  5. Capable both physically and mentally for close work with elderly patients, their families, nursing, and medical staff.
  6. Understand the need for confidentiality of information gained while performing the job.

Performs other reasonable related duties as assigned by immediate supervisor and other management as required.
