We are seeking a skilled Licensed Practical Nurse to join our team at HexaQuEST Health, Inc. in a Long Term Care setting.
Job Summary:The Licensed Practical Nurse in Long Term Care will provide total patient care services as directed, measuring, evaluating, administering, and documenting patient care. The ideal candidate will contribute to patient care plans, inform Registered Nurses when necessary, and provide direction and assign duties to Certified Nursing Assistants in accordance with their skills and abilities.
Responsibilities:- Medication administration, care management, and direct patient care, such as bathing, dressing, and feeding patients, and assisting in examinations and treatments.
- Ability to lift, walk, carry, and stand.
- Oversee total patient care and work directly with other staff to assure provision of highest standards of care.
- Compliance with the State of Nevada's Nurse Practice Act, Hospital policies, and rules/regulations.
- Ability to organize and prioritize work to maintain harmonious daily workflow.
- Check, pour, administer, and document medications at appropriate times, monitor effects and side effects of medications.
- Work with intravenous medications and administer all routes of medications per Nevada State Board of Nursing criteria.
- Administer and record treatments as ordered, monitor progress toward expected outcomes of treatments.
- Admit, transfer, and discharge patients as required.
- Report changes in condition, laboratory results, unexpected occurrences to the RN and the physician.
- Accurate documentation, reporting, and in-depth nursing observation to assure continuity of patient care.
- Daily evaluate patient care needs and condition changes, and implement changes in care delivery as warranted.
- Consult with the RN in implementing such changes.
- Assure that Patient Rights are upheld.
- Transcribe physician orders and communicate changes to other members of the team.
- Assure that Labs, X-rays, etc., are completed and notify attending physician of abnormal results.
- Assist as necessary with patient care.
- Maintain cleanliness and order on unit.
- Participate in and satisfactorily complete the Orientation Program and Annual Review Days.
- Attend inservices and meetings as required.
- Pursue thirty (30) contact hours of continuous education, as required by the State of Nevada for re-licensure.
- Report deviations in quality of care to Supervisors as outlined by current nursing standards.
- Actively participate in Quality Improvement activities.
- Maintain working knowledge of hospital-wide order entry.
- Graduation from a recognized Practical Nurse Program.
- Current licensure by the Nevada State Board of Nursing as an LPN.
- IV certification within one year of employment.
- Neat, well-groomed, professional appearance.
- Capable both physically and mentally for close work with elderly patients, their families, nursing, and medical staff.
- Understand the need for confidentiality of information gained while performing the job.
