Minimum Requirements:
  • Licensed to practice medicine in accordance with Florida Statutes.
  • Four years of relevant experience, with residency completion potentially substituting for required experience.

Preferred Qualifications:
  • Board certification in the relevant subspecialty.
  • Certification/Licensure in Florida and Federal DEA Certification.
  • Medical Degree - M.D., D.O., or equivalent.
  • Completion of an accredited residency program.
