RN (Per Diem/On Call) - PACE/Elderplace

This per diem/on call position supports our Beaverton site and surrounding areas as needed, providing care management, care coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Assist providers in effectively identifying needs and coordinating access to appropriate care and services to improve or maintain the patient's functional status and independence.
- Plan, oversee, and monitor all nursing and unlicensed caregiver services to each patient on their panel within the context of an interdisciplinary team.
- Travel between multiple locations to provide care and services.
- Oregon Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- Relevant certifications applicable to the position, as specified by the Nurse Manager, upon hire.
- Bachelor's Degree in Nursing.
- 1 year Home health RN experience.
