A Home Health LPN at Interim HealthCare will provide home-based care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN, working with a team of physicians, RNs, CNAs, and aides to deliver quality patient care. Key responsibilities include taking patient vitals, conducting assessments, documenting progress, and reporting changes to the RN supervisor, as well as assisting in carrying out patient plans of care and teaching self-care techniques to patients and their families.

For this role, we seek candidates with a graduate degree from an accredited practical nursing program, an active LPN license in Colorado, and a minimum of six (6) months of LPN experience, ideally in home healthcare, or completion of our office's preceptor program. A strong understanding of state and federal home health regulations is essential, along with clinical proficiency in nursing, excellent communication skills, and compassion.
