As a Hospice Nurse Specialist at Gentiva Health Services, you will play a vital role in providing exceptional patient care and support to individuals and their families during challenging times.

Job Description

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. The successful candidate will be responsible for managing a caseload of patients, providing direct care services, and collaborating with an interdisciplinary team to deliver comprehensive care.

Key Responsibilities:
  • Manage patient clinical records and develop individualized care plans
  • Provide emotional support and education to patients and their families
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions
  • Evaluate patient needs and implement effective interventions to promote comfort, peace, and dignity
Requirements

To be successful in this role, you will require:

  • A current RN license in the state of employment (or eligibility to obtain)
  • 1+ years of experience in hospice, home health, ICU, geriatric, or oncology nursing
  • Demonstrated ability to work effectively in a fast-paced environment and provide empathetic care to patients and families
