Job Summary

Responsibilities
  • Conduct comprehensive patient assessments, including functional, mental, social, and physical conditions.
  • Develop and implement treatment plans, evaluating patient outcomes and making adjustments as needed.
  • Provide supervision to clinical staff, ensuring high-quality patient care.
  • Collaborate with the multidisciplinary team to ensure timely and comprehensive services for patients.
  • Participate in process improvement initiatives, contributing to team development and program growth.
  • Actively participate in hospital and department projects and committees as appropriate.
Qualifications

Minimum Qualifications:

  • Current APRN or PA-C licensure in Kentucky.
  • Eligibility for a Medicare provider number and DEA registration.

Preferred Qualifications:

  • Experience in APRN/PA-C patient care.
