Job Overview

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at the Ypsilanti Health Center. This full-time position offers a rewarding opportunity to work in an ambulatory care setting, providing exceptional patient care and contributing to the success of our healthcare team.

 

Key Responsibilities

  • Provide nursing care to patients in accordance with institutional and health center guidelines, prioritizing their physical and emotional well-being.
  • Participate in the planning, implementation, and evaluation of nursing care under the direction of a Registered Nurse, ensuring seamless collaboration and communication.
  • Perform various tasks related to prescription renewal, quality improvement activities, and process improvements, enhancing patient outcomes and clinic operations.
  • Assist with quality care initiatives, staff meetings, and patient phone call management functions, fostering a culture of teamwork and excellence.
  • Collaborate with the Registered Nurse Team to support patient care, troubleshooting issues, and resolving concerns in a timely manner.
  • Maintain accurate and detailed records, adhering to UMHS Policies and Procedures, and utilizing EMR systems effectively.

 

Requirements and Qualifications

  • Certification as a Licensed Practical Nurse (LPN) in the State of Michigan.
  • Graduation from an approved School of Practical Nursing.
  • Experience in patient instruction and telephone interaction with patients, with a strong focus on empathy and effective communication.
  • National Association for Practical Nurse Education & Services (NAPNES) certification or completion of a pharmacology course in an accredited program.
