Job Summary:

Greater Cincinnati Behavioral Health Services (GCBHS) is seeking a highly skilled Senior Case Manager to join our team. As a Senior Case Manager, you will play a critical role in providing community-based mental health services to individuals with severe and persistent mental illness.

Key Responsibilities:

  • Provide case management services to individuals with severe mental illness, including assessment, planning, and coordination of services.
  • Develop and implement individualized treatment plans to help clients achieve their goals and maximize independence.
  • Collaborate with other service staff, including psychiatrists, therapists, and support staff, to ensure comprehensive care.
  • Conduct regular assessments and evaluations to identify client needs and develop strategies to address them.
  • Provide education and support to clients and their families to promote understanding and empowerment.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Participate in on-call rotation and provide crisis intervention services as needed.
  • Document all client interactions and services provided in accordance with GCBHS policies and procedures.

Requirements:

  • Bachelor's or Master's degree in a Human Services field or High School diploma and a minimum of three years experience in Behavioral Health, Mental Health, or a Human Services related field.
  • Two years of experience as a Mental Health Care Manager or in a related field.
  • Experience in social services and behavioral health.
  • Frequent driving of the employee's personal vehicle is required. A valid driver's license, vehicle, and insurable driving record are required.
