Job Summary

The Transfer Center Registered Nurse is responsible for facilitating the transfer of patients from one healthcare facility to another. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Collects and analyzes clinical information to determine the appropriateness of patient transfers
  • Facilitates communication between healthcare providers and facilities to ensure smooth transfers
  • Assesses patient needs and develops plans to ensure timely and effective transfers
  • Collaborates with interdisciplinary teams to ensure seamless transitions of care
  • Manages multiple phone lines and prioritizes incoming calls to ensure timely responses
  • Documents all communication and transfer activities in a timely and accurate manner
Requirements
  • Registered Nurse licensure in the State of Florida or eligible compact state
  • 5 years of nursing experience in an acute care hospital
  • Basic Life Support (BLS) certification
  • Excellent communication and interpersonal skills
  • Strong organizational and problem-solving skills
Preferred Qualifications
  • Bachelor's degree in Nursing
  • Experience in transfer center or similar role
