Bilingual Community Care Coordinator

This role is primarily field-based, requiring candidates to work within the community. Candidates must reside in Connecticut to be eligible.
Target Areas:
Hartford, Windsor Locks, Manchester, Norwich, Middletown, Windham, Waterbury, Torrington, and Danbury, CT.
Work Schedule:
Monday to Friday, 9 AM to 5 PM. Flexibility to accommodate the needs of families is essential, including potential work outside of standard hours.
The Outreach Care Specialist is responsible for providing individual and family support to ensure that members are connected to community services, resources, and effective care coordination.
Collaboration with ICC staff, families, consumers, community partners, stakeholders, and service providers is crucial to ensure that appropriate services are accessible to designated members.
The primary objective is to enhance independence and quality of life by assessing individual needs and facilitating access to suitable community services and supports.
Key Responsibilities:
1. Coordinate follow-up care plans for members by scheduling appointments or enrolling them in relevant programs.
2. Assess member adherence to medical treatment plans through telephone communication or on-site visits.
3. Identify barriers to compliance and facilitate resolutions.
4. Recognize opportunities impacting quality goals and suggest process enhancements.
5. Recommend modifications to treatment plans and determine the necessity for additional services in collaboration with case management and providers.
6. Facilitate identification and referral to local, state, or federally funded programs.
7. Coach members on strategies to mitigate health risks.
8. Prepare documentation to report case and compliance updates.
9. Establish and maintain relationships with relevant agencies as outlined in contracts.
10. Perform other assigned duties as needed.
Minimum Qualifications:
High school diploma and a minimum of one year of relevant experience, or a combination of education and experience that provides an equivalent background.
Preferred Skills and Experience:
1. Familiarity with community resources in the area.
2. Bilingual (Spanish) candidates are preferred.
3. A bachelor's degree in a related field is preferred.
4. Experience with electronic health records (EHR) systems.
For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to possess strong oral, written, and interpersonal communication skills, along with problem-solving, facilitation, and analytical skills.
