Our clinicians visit patients in their homes, providing a continuum of care for those with chronic conditions and limited mobility. We offer a range of programs, including remote patient monitoring, behavioral health management, and chronic care management, to ensure our patients receive the highest quality of care from a team they trust.

We are seeking a Full-time, experienced Community Liaison for our Fairfax / Arlington, VA region. The Community Liaison will be responsible for interacting with the community and promoting our house call and hospice services.

Key Responsibilities
  • Analyze the potential of our service area to determine target markets.
  • Visit healthcare facilities, such as doctor's offices, hospitals, and skilled nursing facilities, to present our credentials and obtain patient referrals.
  • Analyze our organization to determine its strengths and weaknesses.
  • Analyze past and current marketing data.
  • Complete an analysis of our service offerings.
  • Analyze patient relationships.
  • Develop sales and marketing objectives and projections.
  • Develop a marketing plan, identifying priorities and setting a reasonable timetable.
  • Implement the marketing plan, staying within the established timetable.
  • Review and evaluate the analyses and plan on a regular basis.
  • Attend meetings as appropriate.
  • Assist office staff as needed.
  • Provide community outreach and education.
Requirements
  • At least 3 years of experience in marketing, nursing, or social work interacting with healthcare agencies and professionals.
  • Must be organized and detail-oriented.
  • Must be able to communicate effectively orally and in writing.
  • Must be a licensed driver with an insured automobile in good working order.
