Job Overview:

OBJECTIVE OF POSITION:
To enhance patient care quality; document and maintain healthcare standards while reporting findings to track statistical anomalies.

Collect and manage information efficiently and effectively to promote the health of our patients and communities, addressing the requirements of regulatory bodies (both public and private) while optimizing available incentives by achieving the outlined objectives of these oversight organizations.


KEY RESPONSIBILITIES:
1. Establish and uphold quality benchmarks necessary for achieving recognitions that define primary care excellence. 2. Educate, support, and guide all team members on the appropriate documentation practices related to the aforementioned recognitions. 3. Facilitate seamless transitions of care through effective communication with patients, caregivers, healthcare providers, and facilities, adhering to established policies, guidelines, treatment plans, and standing orders. 4.

Coordinate appointments in line with care standards, utilizing guidelines to identify and address care gaps, while providing support and education on the self-management of chronic conditions as outlined in the physician's treatment plan based on evidence-based practices and population health strategies.

5. Organize team meetings with physicians and clinical personnel to review high-risk patients; enhance the roles of medical assistants and other clinical staff through team-based services and initiatives aimed at organizing and sustaining high-quality patient care.


Position Requirements:

EDUCATION:

High school diploma, medical assistant certification from an accredited vocational institution, or completion of a community college course in medical assisting.


Current medical assistant certification is preferred.
EXPERIENCE:
Minimum of 3 years of recent experience in a medical setting as a medical assistant is preferred. Experience in a primary care office is advantageous.

SKILLS:
Proficiency in Microsoft Office is required. Current health records with necessary immunizations to work in the healthcare sector (hepatitis B and tuberculosis).

KNOWLEDGE:
Understanding of healthcare practices and medical office protocols/procedures. Familiarity with specific assisting tasks relevant to particular medical specialties. Awareness of information that must be communicated to patients and their families.

SKILLS:
Strong communication abilities. Skill in tact and diplomacy during interpersonal interactions. Proficiency in identifying patient education needs and effectively conveying information to patients and families.

ABILITIES:
Capacity to learn and retain information regarding patient care protocols. Ability to present a pleasant and professional demeanor. Competence in planning, prioritizing, and completing assigned tasks. Ability to demonstrate empathy and compassion in interactions with others.
