Overview

CHI Health St. Francis is a prominent healthcare institution dedicated to enhancing community health. Our facility is a regional referral center, employing over 1,100 staff members and collaborating with more than 100 physicians to deliver exceptional medical services.

Our mission is to ensure that patients receive top-notch healthcare in a familiar environment, supported by their loved ones and community. The inpatient rehabilitation unit, located on the sixth floor, comprises 13 beds and specializes in enhancing patients' mobility and independence through comprehensive therapies, including physical, occupational, and speech therapy.

We cater to individuals recovering from various conditions, such as strokes, fractures, arthritis, cardiac surgeries, and neurological disorders like Parkinson's disease.

Key Responsibilities

Under the guidance of a Registered Nurse or Licensed Practitioner, you will engage in the nursing process, which encompasses the following phases:

1. Assessment - Utilize a systematic approach to gather and evaluate healthcare consumer data, considering psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors. You will contribute to the initial assessment conducted by the RN and may perform re-assessments as necessary.

2. Planning - Establish measurable and attainable short- and long-term objectives based on the RN's assessment, collaborating with the healthcare consumer and the multidisciplinary team to develop and adjust the care plan.

3. Implementation - Execute and/or adapt the care plan utilizing assessment data and clinical reasoning in accordance with established standards of care and evidence-based practices. This includes care coordination, patient education, and discharge planning. You will be accountable for the care provided, whether directly or through delegation, adhering to professional and legal standards.

4. Evaluation - Employ a systematic, ongoing process to assess progress toward identified healthcare consumer goals. Revise the care plan as needed, involving the healthcare consumer and the multidisciplinary team in the process.

Document all stages of the nursing process in the healthcare consumer's record.

Qualifications

- Graduate of an accredited LPN nursing program
- Active LPN licensure in the state of Nebraska or nurse licensure compact
- Basic life support certification from the American Heart Association

If employed in a Nebraska facility, completion of an approved LPN-IV therapy course is required if the LPN program was completed outside of Nebraska or prior to 2016, or validation of prior LPN-C is required upon hire.

Compensation

Competitive pay rate of $28.35 per hour.
