NYC Health + Hospitals/Gotham Health is seeking a highly skilled and compassionate Nurse Practitioner to join our team in Ambulatory Medicine. As a key member of our healthcare team, you will provide high-quality patient care, education, and support to our patients and their families.
Job Summary:The Nurse Practitioner will work under the general administration of the Director of Nursing and in collaboration with a physician and accountability to the Medical Director and/or designee. The successful candidate will assume a comprehensive and responsible function in primary health care for adults aged 18 and older.
Responsibilities:- Provides medical services within the scope of approved privileges
- Documents all patient contact in the EMR in a timely manner
- Assumes responsibility for their panel of patients and those patients they care for
- Collaborates to provide care coordination of activities in providing comprehensive care
- Provides relevant health education and instruction; counsels patient/family/responsible other on the impact of illness and principles of health promotion and maintenance
- Participates in departmental meetings and interdisciplinary or community committees, as well as Peer Review Performance Improvement meetings and activities, as required
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED)
- Current certificate as a Nurse Practitioner issued by the NYSED
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA)
- Satisfactory completion of an approved Nurse Practitioner graduate course consisting of classroom and clinical experience and two (2) years of satisfactory advanced clinical nursing experience
- Certified and Licensed as Adult Health Nurse Practitioner by New York State Department of Education
