Clinical RN Care Coordinator

As a Clinical RN Care Coordinator for Anticoagulation at Providence, you will play a vital role in delivering high-quality, patient-centered care to individuals on anticoagulation medication. You will work collaboratively with healthcare professionals to assess patients, make dosage adjustments, and provide education to patients and their families.
Key Responsibilities- Assess patients on anticoagulation medication and make dosage adjustments based on Point of Care Testing (POCT) or laboratory values.
- Document patient information in Epic and provide detailed ongoing education to patients, family members, and healthcare professionals.
- Collaborate with healthcare teams to ensure high-quality, cost-effective, and anticoagulation drug therapy.
- Graduate of an accredited RN nursing program.
- Montana Registered Nurse License.
- National Provider BLS - American Heart Association.
- 2 years of Clinical RN experience.
- Bachelor's Degree in Nursing.
- Certification in CACP, CVRN, or Med-Surg.
- Recent Anticoagulation Management, Cardiology, or Med/Surg experience.
