Bilingual Nurse

We are seeking a highly skilled and experienced Bilingual Registered Nurse to join our team at 1st Choice, LLC. As a Bilingual Registered Nurse, you will play a critical role in providing high-quality patient care and support to our clients.
Responsibilities:
- Conduct health coaching conversations with patients, taking into account their psychological profile.
- Adhere to HealthForward inc. processes and program-specific processes.
- Oversee and supervise the care provided to patients by Licensed Practical Nurses (LPNs) and Paraprofessionals through regular home visits.
- Act as the coordinator of care for the Multidisciplinary team by providing care to patients.
- Transport and utilize required medical equipment and supplies.
- Report any changes in the client's condition to the Clinical Manager and physician.
- Work under the direction of the Director, managing multiple tasks independently in a fast-paced environment.
- Apply advanced knowledge to conduct multispecialty subjective assessments to evaluate the skills of nurses supporting the TRICARE - Global Nurse Advice Line (GNAL).
- Clearly and completely document audit findings within the audit tracking tool.
- Provide information and guidance and care coordination to the patient for effective care transition, improving self-management skills, promoting post-hospital discharge follow-up, and maintaining patient safety in the community.
Qualifications:
- Must have passed the RN licensing exam and be currently in good standing with the appropriate state Department of Health Professions.
- Minimum of 2 years of previous RN experience required.
- Must be willing to travel to Washington DC, Greenbelt, Prince Frederick, Rockville, Silver Spring, and Waldorf.
- Associates (ASN) or Bachelors degree (BSN) with an active license in the state of practice.
- Flexibility and adaptability as it relates to schedules, responsibilities, and priorities.
