Case Manager

We are seeking a compassionate and driven Case Manager to join our team and make a meaningful difference in the lives of brain injury survivors in the Roanoke Valley.

Responsibilities
  • Provide individualized case management services to clients, including needs assessments, goal setting, and service coordination.
  • Develop and implement comprehensive care plans tailored to each client's unique circumstances and goals.
  • Connect clients with community resources and support systems to facilitate their recovery and well-being.
  • Advocate for clients' needs and rights within the healthcare system and community.
  • Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with all applicable regulations.
Qualifications
  • Bachelor's degree in social work, counseling, human development, therapeutic recreation, or a related field is required.
  • Strong verbal and written communication skills, including proficiency in Microsoft Office Suite.
  • Experience working with individuals who have experienced brain injury or other disabilities is preferred but not required.
  • Valid driver's license and willingness to travel within the Roanoke Valley.
