RN Case Manager

As a RN Case Manager, you will play a critical role in providing case management for assigned patient populations, utilizing your clinical expertise, communication, and problem-solving skills to achieve optimal clinical and resource outcomes.
Key Responsibilities:- Collaborate with Utilization Review Nurse to ensure seamless care transitions.
- Facilitate an interdisciplinary approach to patient care, coordinating services within the case-managed population.
- Perform face-to-face assessments of patients/families to identify individualized needs, in collaboration with Social Workers.
- Document patient care and interventions in the medical record, ensuring compliance with CMS regulations.
- Facilitate referrals for high-cost medications, working with other disciplines to obtain prior authorizations and co-pay information.
- Ensure coordination of care during patient transfers, communicating with outside nursing or case management staff as needed.
- Advocate for patients and families throughout the episode of care.
- Participate in departmental and system performance improvement initiatives.
- Current licensure in Virginia as a Registered Nurse.
- Three years of recent experience in a clinical healthcare setting, with responsibilities reflecting direct management of patient care.
- Supervisory or leadership experience is preferred.
