Job Title: Licensed Practical Nurse Assessment Coordinator (LPNAC)

Job Summary:

Hamilton Arms Center is seeking a highly skilled Licensed Practical Nurse Assessment Coordinator (LPNAC) to join our team. As an LPNAC, you will be responsible for conducting and coordinating the completion of resident assessments in accordance with state and federal regulations. You will also develop and implement quality assurance programs to ensure the accuracy and completeness of resident assessments.

Responsibilities:

  • Conduct and coordinate the completion of resident assessments in accordance with state and federal regulations.
  • Develop and implement quality assurance programs to ensure the accuracy and completeness of resident assessments.
  • Participate in facility surveys and monitor quality improvement and quality management reports.
  • Provide education and training to the interdisciplinary team involved in the resident assessment process.

Requirements:

  • Must be a Licensed Practical Nurse (LPN) with effective leadership and assessment skills in a long-term care setting.
  • Must have previous experience as a LPNAC and assessments.
  • Must be proficient in PA CMI and have knowledge of state and federal regulations.
  • Must have Medicare billing knowledge and experience.
  • Must have knowledge of PPD (PDPM) and insurance updates.
