**Job Summary**

Community Health Centers of the Rutland Region is seeking a skilled Family Medicine Physician to join our team in Rutland, Vermont. As a primary care physician, you will be responsible for delivering high-quality patient care, promoting wellness behaviors, and collaborating with mid-level providers and nursing staff.

**Responsibilities**

  • Empower patients to take an active role in health maintenance and promote wellness behaviors
  • Provide appropriate patient follow-up and communicate effectively with patients
  • Refer patients to specialty care as necessary
  • Provide clinical supervision to mid-level providers and nursing staff
  • Participate in quality assurance activities
  • Participate in State and National disease collaborative efforts

Our Rutland clinic provides comprehensive primary care services, including on-site behavioral health providers, social workers, and care management.
