We are Plantation FL HOS – A collaborative network of Physicians and Nurse Practitioners dedicated to serious illness and end-of-life care. Our services span the entire continuum, from home care and virtual consultations to skilled nursing facilities and acute care hospitals. We deliver care where it is most needed.

Responsibilities

Palliative Care Nurse Practitioner Responsibilities

  • Manage symptoms for patients with serious illnesses.
  • Establish care objectives.
  • Educate patients, families, and the community on how palliative services enhance the quality of care for those pursuing active treatment or in the later stages of serious illness.

Nurse Practitioner Responsibilities

  • Coordinate with the healthcare team to ensure timely visits that comply with regulatory requirements.
  • Provide symptom management for patients with serious illnesses.
  • Offer support to patients and families during home visits.

Qualifications

Qualifications:

  • Master's degree in nursing, required
  • Current and unencumbered advanced practice nursing license in the practicing state(s)
  • Valid CPR certification required
  • Certification from a nationally recognized advanced nursing practice organization
  • Minimum one year of experience in California, required
  • Previous experience in hospice, home health, or specific patient populations (e.g., geriatrics, pediatrics)
  • At least three years of experience in Palliative Care services
  • Familiarity with agency policies and state and federal regulations related to nursing services
  • Demonstrated expertise in pain and symptom management and advanced care planning
  • Must possess a reliable vehicle, valid driver's license, and the minimum state-required liability auto insurance
