Key Responsibilities

* Provide palliative care to patients with serious illnesses, focusing on symptom management and quality of life
* Work collaboratively with our medical team to develop and implement individualized care plans
* Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
