Responsibilities:
  • Assess patients' needs and develop individualized care plans
  • Provide education to patients and their families on palliative care and disease management
  • Work closely with physicians and other healthcare professionals to ensure seamless care coordination
  • Participate in multidisciplinary team meetings to discuss patient care and develop treatment plans
  • Document patient information and care plans in our electronic health record system
Requirements:
  • Graduate of an accredited Physician Assistant program
  • Experience working in a hospital-based palliative care setting
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Florida license to practice as a Physician Assistant
