Clinical Pharmacist II

We are seeking a highly skilled Clinical Pharmacist II to join our team at CHRISTUS Health. As a key member of our pharmacy department, you will play a vital role in providing exceptional patient care and contributing to the advancement of our pharmacy services.
Key Responsibilities- Prepare and dispense drug orders according to established policies and protocols.
- Interpret and transcribe drug orders, maintaining accurate patient medication records.
- Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products.
- Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
- Supervise and direct pharmacy support personnel, verifying daily activities and maintaining competency.
- Participate in quality improvement and medication use review activities, collecting data and conducting monitors and inspections.
- Conduct designated interventions, including IV to PO conversions, renal dosing, and pharmacokinetic dosing support.
- Attend interdisciplinary rounds and contribute to the advancement of the pharmacy department through precepting, competency assessment, and in-service training.
- Doctor of Pharmacy or BS Pharmacy degree required.
- One of the following: BPS Board Certification, Residency, Fellowship, or at least 5 years of hospital experience.
- PGY-1 and PGY-2 residency completion, Board certification, or 5 years hospital experience required at hire.
- Pharmacy registration in the State of practice.
- Must have pharmacy State licensure within 60 days of start date.
- BLS or ACLS and PALS (pediatric practice) within 180 days of start date.
