Specialty Clinical Pharmacist II

The Specialty Clinical Pharmacist II is a key member of the healthcare team, responsible for providing safe, effective, and prompt medication therapy to patients. This role requires a high level of clinical expertise, strong communication skills, and a commitment to delivering exceptional patient care.
Responsibilities- Provide direct patient care, including assessing patient needs, developing and implementing medication therapy plans, and monitoring patient response to therapy.
- Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives to enhance medication use processes and patient safety.
- Develop and maintain expertise in specialty medication therapy management, including Hepatitis C, inflammatory conditions, pulmonary conditions, neurology, dyslipidemia, osteoporosis, and oncology.
- Provide education and training to healthcare providers, patients, and families on medication therapy and disease management.
- Conduct clinical research and practice advancement projects to improve medication use processes and patient outcomes.
- Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy.
- PGY1 residency or 5 years of clinical practice experience required.
- PGY2 residency preferred.
- Specialty pharmacy experience preferred.
- EPIC suite products experience preferred.
- Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine.
- Licensed pharmacist in the state of practice.
- Pharmacy preceptor license within 90 days of start date.
- Must accrue ASHP Residency Preceptor status within 18 months of hire.
- Must obtain Certified Specialty Pharmacist (CSP) certification or other applicable board certification within 18 months of hire.
