Job Details

Full-time position available for our Kent Medical Clinic. The Care Transition Specialist will work closely with patients, families, and healthcare providers to coordinate care, address barriers to access, and promote healthy behaviors.

About the Role

This role requires strong critical thinking skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. The Care Transition Specialist will work collaboratively with the healthcare team to identify and address gaps in care, provide education and support to patients and their families, and coordinate care transitions to ensure seamless continuity of care.

Responsibilities
  • Conduct comprehensive assessments of patient needs and develop individualized care plans
  • Collaborate with healthcare teams to identify and address gaps in care
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
  • Coordinate care transitions to ensure seamless continuity of care
Requirements

To be successful in this role, you will need:

  • A Medical Assistant diploma or equivalent experience
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Knowledge of medical terminology and healthcare systems
