Through a diverse range of residential and non-residential services, BFL supports disabled survivors of domestic violence, the homeless disabled population, and individuals facing mental health or substance use challenges.
BFLA (BFL Apartments) is a NY/NY III supportive housing program offering vital services to vulnerable populations, including homeless disabled adults, disabled veterans, and their families. Located in the South Bronx, BFLA comprises two buildings: a 70-unit studio apartment building for single adults (SPMI-chronic homeless population A) and a 50-unit apartment building for families (SPMI/MICA-chronic homeless population G).
We are seeking a dedicated and compassionate Behavioral Health Coordinator to join our team. Reporting to the Director of Social Services, you will play a crucial role in supporting tenants and fostering their well-being.
Responsibilities:- Manage a caseload of 10-15 SPMI/MICA tenants, providing individualized support and guidance.
- Conduct comprehensive program intakes and assessments for new tenants, identifying their needs and goals.
- Develop, monitor, and update service plans in collaboration with each tenant, ensuring they are tailored to their unique circumstances.
- Facilitate regular follow-up meetings with tenants, including home visits, to discuss progress, address challenges, and reinforce support strategies.
- Provide individual supportive counseling sessions to assist tenants in developing essential skills for stabilizing their medical, psychosocial, and behavioral health.
- Offer crisis intervention services and equip tenants with effective coping mechanisms to navigate difficult situations.
- Collaborate closely with the interdisciplinary team to implement comprehensive strategies that support tenant success and well-being.
- Accompany tenants to court hearings (Family/Criminal/Housing) and advocate for their needs as required.
- Provide referrals to legal assistance and other community resources as necessary.
- Assist tenants with any ACS or Prevention services requirements.
- Conduct ongoing apartment inspections, tobacco assessments, and vocational assessments as mandated by the DOHMH contract.
- Monitor tenants' ability to manage rent and utility payments, ensuring housing stability.
- Participate actively in staff meetings, case conferences, supervision sessions, and required training programs.
- Ensure timely and accurate documentation of progress notes within AWARDS.
- Adhere to all agency and program policies and procedures.
We are seeking a highly motivated and compassionate individual with a strong commitment to supporting individuals facing challenges. A Bachelor's degree in Social Work, Psychology, or a related field is required. Experience working with vulnerable populations, particularly those experiencing homelessness or mental health issues, is highly desirable.
