Job Summary:We are seeking a highly skilled and compassionate Registered Nurse to join our home health team as a PM RN Case Manager. In this role, you will be responsible for managing a caseload of patients in specialty areas, conducting initial assessments, and developing and implementing individualized care plans. You will work closely with our interdisciplinary team to ensure seamless care coordination and provide exceptional patient and family-centered care.

- Collaborate with the interdisciplinary team to ensure seamless care coordination and provide exceptional patient and family-centered care.
- Conduct ongoing assessments of patient response to treatments and teaching, and apply interventions as needed.
- Update and revise care plans as indicated, and complete documentation accurately and promptly.
- Communicate with physicians to confirm and update patients' plans of care, and provide status updates to ensure ongoing communication.
- Facilitate discharge planning according to standards, and utilize resources to ensure continuity of care after discharge.
- Participate in chart audits and continuous quality improvement activities as requested, and identify opportunities for process improvement.
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months.
- Valid driver's license issued by the Division of Motor Vehicles.
- Associate's Degree in Nursing.
- Typically requires 3 years of experience in clinical nursing.
This role requires the ability to move about in confined spaces, including bending, twisting, kneeling, squatting, and occasionally reaching overhead. You must be able to work safely with people of various sizes and weight with diverse and complex medical conditions and physical limitations. You will also need to sit, stand, walk, and climb stairs for limited to prolonged periods of time to complete required paperwork, gain entry to patients' residences, travel from assigned visits, and attend work-related activities.
You must have the physical stamina to attend to patient care requirements over a period of three or more hours without complete rest. You will also need to have accurate vision to read gauges and measuring devices and to interpret written instructions.
This role is subject to extreme temperature changes due to weather conditions, exposure to chemical and hazardous waste, and communicable disease.
You must be able to lift up to 50 pounds from the floor to waist, lift up to 20 pounds from waist to overhead, and lift and carry up to 40 pounds at waist height for a reasonable distance.
You must also be able to push/pull with 30 pounds of force and perform a sliding transfer of 150 pounds with a second person present.
