We are seeking a skilled Home Health RN Case Manager to join our team in the Northwest Milwaukee area. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and managing caseloads in specialty areas.
Key Responsibilities:
- Conduct thorough initial assessments to obtain physical, social, and environmental data
- Establish and implement plans of care in accordance with physician orders and patient needs
- Provide direct patient care and adjust nursing care processes as needed
- Collaborate with family and other healthcare professionals to ensure comprehensive care
- Document patient information accurately and promptly
Requirements:
- Registered Nurse license issued by the state in which you practice
- Basic Life Support (BLS) certification
- Valid driver's license
- Associate's Degree in Nursing
- 3 years of experience in clinical nursing
Working Conditions:
This role requires the ability to work safely with patients of various sizes and weight, with diverse and complex medical conditions and physical limitations. You must be able to sit, stand, walk, and climb stairs for prolonged periods and have the physical stamina to attend to patient care requirements over several hours.
