Job Description

**Job Summary:**

We are seeking a highly motivated and compassionate Community Outreach Coordinator to join our team at Step Up. As a key member of our outreach team, you will work closely with individuals experiencing chronic homelessness and mental health conditions in the city of Beverly Hills.

Key Responsibilities:

  • Provide outreach services to engage homeless individuals with chronic mental illness and provide necessary referrals to Los Angeles' most competitive and comprehensive Mental Health, Veterans, and Housing programs.
  • Work as part of a multi-disciplinary team to serve adults on the streets of Beverly Hills and local surrounding community.
  • Collaborate with licensed staff to further evaluate for a qualifying mental illness diagnosis and develop rapport with individuals to promote linkage to services.
  • Assist with benefits establishment and attend staff meetings, supervision, and trainings as requested.
  • Document and collect data through the administration and input of CES packets and maintenance of clear and timely submission of Community Outreach Services (COS) notes via Apricot HER system.

Requirements:

  • BA in psychology or social work, or 2 years of related work experience, with a minimum of 2 years' experience working with persons experiencing mental health disorders and homelessness, in a community setting.
  • Case management experience is essential.
  • Candidate must have own transportation and present evidence of current California driver's license and insurance.
