Job Summary

We are seeking a highly skilled and compassionate Case Manager III to join our team at the Society of St. Vincent de Paul South Pinellas. As a key member of our Homeless Services department, you will play a vital role in providing assessment, monitoring, planning, linkage, and advocacy for individuals and families enrolled in our rapid rehousing services.

Your primary responsibility will be to ensure that our clients receive the most appropriate services to achieve housing stability and independence. You will work closely with our team to develop and implement individualized housing stability plans, provide crisis intervention, and connect clients with community resources.

Key Responsibilities
  • Screening households to determine eligibility and document assessment results
  • Conducting assessments to identify individual and family strengths, needs, abilities, and preferences
  • Developing and implementing housing stability plans, including crisis intervention and barrier identification
  • Monitoring progress and maintaining accurate documentation of client services and outcomes
  • Collaborating with community partners to provide wrap-around services and support
  • Providing referrals and facilitating access to resources and services as needed
  • Conducting home visits and maintaining accurate records
  • Ensuring compliance with regulatory requirements and agency policies
Requirements
  • Bachelor's degree in social work or related field
  • Minimum 2 years of experience serving homeless or at-risk populations
  • Knowledge of homelessness, severe and persistent mental illness, and substance abuse
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks
  • Excellent communication and interpersonal skills
  • Valid Florida driver's license and reliable transportation
