About the Role

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. As a Private Duty Nurse, you will provide high-quality, one-on-one care to our patients in the comfort of their own homes.

Responsibilities
  • Implement and manage patient care plans in accordance with physician orders and Aveanna Healthcare policies.
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes.
Requirements
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • One year of experience as a LPN/LVN in a clinical care setting (preferably in home health or pediatrics).
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
