Responsibilities- Implement and manage patient care plans in accordance with physician orders and Aveanna Healthcare policies.
- Conduct thorough patient assessments and coordinate care with other healthcare professionals as needed.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
- Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes.
- Current, unrestricted LPN/LVN license in the state of practice.
- Current CPR certification from the American Red Cross or American Heart Association.
- One year of experience as a LPN/LVN in a clinical care setting (preferably in home health or pediatrics).
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
