Job Summary:

We are seeking a highly skilled and compassionate Social Worker to join our team at Shriners Children's Chicago. As a Social Worker, you will play a vital role in providing assessments, planning, interventions, and follow-up services to patients and their families across the continuum of care.

Responsibilities:

  • Conduct comprehensive assessments to identify patient and family needs
  • Develop and implement individualized plans to achieve desired outcomes
  • Collaborate with healthcare teams to coordinate resources and services
  • Advocate for patients and families to ensure their health, safety, and rights are protected
  • Arrange services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care

Qualifications:

  • Masters in Social Work
  • Illinois Social Work license
  • Case Management Certification (CMC, ACM, or ANCC) required or the ability to attain certification within 12 months of hire
  • Three years of social work experience

Preferred Qualifications:

  • 5-7 years of social work experience
  • Previous social worker experience in a pediatric setting
  • Bilingual in English/Spanish
