Job Title: Licensed Practical Nurse - Hospice Care

We are seeking a skilled Licensed Practical Nurse to join our team at Memorial White Rose Hospice in York, PA. As a member of our team, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Responsibilities:
  • Perform skilled nursing services under the supervision of a staff RN daily.
  • Administer prescribed medications and treatments as permitted by state regulations.
  • Assist the physician and/or staff RN in performing specialized procedures as needed.
  • Assist the patient with daily activities and encourage appropriate self-care daily.
  • Report to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed.
  • Perform complex wound care if in-service is documented for specific procedure daily or as needed.
Requirements:
  • Associate Degree or certification equivalent.
  • Unrestricted licensure in state of practice.
  • Current CPR certification required.
  • Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.
