Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a compassionate and skilled Private Duty Nurse LVN to join our team.

About the Role

As a Private Duty Nurse LVN, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing continuous care to remain at home with their loved ones. You will work closely with our patients, their families, and our healthcare team to ensure the best possible outcomes.

Responsibilities
  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease management
  • Accept and write verbal orders from physicians as required
  • Follow up with patients, families, and healthcare team on client problems and concerns
Qualifications
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
