Medical Care Coordinator

We are seeking a compassionate and motivated Registered Nurse with a wound focus to join our skilled nursing team as a Medical Care Coordinator - Skin Health Specialist. This is a full-time position offering a competitive salary of $34-$37 per hour, plus benefits.
About the RoleIn this critical role, you will lead and integrate a comprehensive Skin Health and Wound management program, collaborating with our nursing center management team to ensure high-quality patient care. Your expertise will be crucial in supporting central supply and overseeing durable medical equipment related to wound prevention and management.
Key Responsibilities- Develop and implement a fully developed Skin Health and Wound management program, working closely with nursing center management.
- Collaborate with our clinical team to provide education on skin, wound, and incontinent products.
- Oversee the use of durable medical equipment for wound prevention and management.
To succeed in this role, you must possess:
- A valid RN license (MPT/DPT with leadership review/approval).
- Ability to complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
- Commitment to attend monthly support calls.
- National accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or commitment to complete Advanced Wound Training and national accredited Wound Certification within one year of hire.
- Strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
- Experience in team building and demonstrating respect for the interprofessional team in wound management.
- Knowledge of national wound guidelines and wound products.
- Knowledge of nursing home regulations and survey process.
- Strong Microsoft Word, Excel, Database Management, PowerPoint skills.
